Responsibilities
Provide skilled nursing care to patients in their homes.
Perform comprehensive patient assessments and develop individualized plans of care.
Complete OASIS assessments accurately and in accordance with CMS and agency requirements.
Manage patient cases and coordinate care with the interdisciplinary team.
Monitor patient progress and communicate changes in condition to physicians and other appropriate providers.
Provide medication management, patient education, disease management, and health promotion.
Educate patients and caregivers regarding treatment plans, medications, safety, and disease processes.
Document all patient care accurately and timely in the electronic medical record.
Coordinate admissions, recertifications, resumptions of care, transfers, and discharges as applicable.
Collaborate with physicians and other healthcare professionals to ensure continuity of care.
Maintain compliance with agency policies, state regulations, and applicable Medicare/CMS requirements.
Provide compassionate, patient-centered care while promoting patient independence and quality of life.
Requirements
Active Vermont RN license required.
Minimum 1+ year of recent Home Health RN experience.
Previous experience completing OASIS assessments required.
Home Health Case Management experience required.
Previous experience with Homecare Homebase (HCHB) EMR required.
Strong EMR documentation experience required.
BLS/BCLS certification required.
Must be comfortable working independently in a home health setting.
Strong assessment, communication, organizational, and care coordination skills required.
Valid driver's license and reliable transportation required for travel between patient visits.
Must be able to work the Tuesday–Saturday schedule.
EMR Requirement

Homecare Homebase (HCHB) experience is required. Candidates must have prior hands-on experience documenting home health visits, assessments, care plans, and related clinical information in HCHB.

OASIS Requirement

Recent OASIS experience is required. Candidates must be comfortable completing accurate OASIS assessments and supporting documentation in accordance with current home health and Medicare requirements.
