Description

Job Description – Home Care Registered Nurse (RN)

The Home Care Registered Nurse is responsible for providing skilled nursing care to patients in their homes while serving as the primary case manager. The RN assesses patient and caregiver needs, develops individualized plans of care, provides nursing interventions, and evaluates patient outcomes in accordance with evidence-based practice, federal and Vermont state regulations, and agency policies.

The RN will complete and maintain accurate OASIS documentation, coordinate care with physicians and interdisciplinary team members, and ensure timely and compliant documentation in the McKesson EMR. The position requires strong knowledge of home health nursing, including high-tech skills such as IV therapy, wound VAC management, and complex wound care.

The RN will educate patients and caregivers regarding medications, treatments, discharge instructions, reportable conditions, and available services. The nurse will also coordinate physical therapy, social work, home care assistance, equipment/vendors, transfers, and safe patient discharges.

The position requires collaboration with LPNs, delegation and supervision within the LPN scope of practice, participation in case conferences and peer-review audits, and proactive advocacy for patient and family needs. Hospice coverage is required while on call.

Position Requirements
Registered Nurse with a minimum of 2 years of experience
Home Health RN experience
OASIS experience required BLS, ACLS, or OASIS certification.
Ability to work independently in a home-care setting
Strong assessment, case management, documentation, and care coordination skills
Experience with high-tech nursing skills such as IV therapy, wound VAC, and complex wound care preferred/required as applicable
Current Vermont RN license required upon acceptance
Ability to work with the McKesson EMR
Willingness to participate in on-call, weekend, and holiday rotations
Schedule

Monday–Friday, 8:00 AM–4:00 PM, with required on-call, weekend, and holiday rotation.
Patterns Hiring Job ID #298982. Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
