Position Overview

Tide Health Group is seeking an experienced Occupational Therapist (OT) for a 13-week Long Term Care assignment in Norman, OK. The OT will evaluate patients with cognitive and/or physical functioning limitations and provide skilled, medically necessary rehabilitation services to improve functional independence and activities of daily living.

Key Responsibilities
Screen and evaluate patients with cognitive and/or ADL-related physical functioning disorders.
Develop individualized plans of care in accordance with clinical and regulatory standards.
Provide skilled occupational therapy and medically necessary rehabilitation interventions.
Perform ADL retraining, therapeutic activities, therapeutic exercises, and appropriate modalities.
Recommend and utilize supportive and assistive devices as clinically appropriate.
Monitor patient progress and modify treatment plans based on clinical response.
Evaluate treatment outcomes and make referrals when appropriate.
Provide education, consultation, and counseling to patients, families, caregivers, and other service providers.
Communicate patient progress, barriers, and treatment plans with supervisors and interdisciplinary team members.
Complete comprehensive discharge summaries.
Maintain accurate electronic clinical documentation and patient records.
Accurately record treatment minutes.
Participate in patient care conferences, utilization review meetings, rehabilitation meetings, and other required meetings.
Participate in required facility/company training, in-services, and conferences.
Obtain necessary DME and rehabilitation supplies.
Supervise and support Occupational Therapist Assistants, rehabilitation aides, and/or students as applicable.
Practice in accordance with NBCOT standards, Oklahoma regulations, professional ethics, and applicable practice requirements.
Required Qualifications
Minimum 1+ year of OT experience
LTC or Rehabilitation experience required
Active Oklahoma OT license required
New graduates may be considered if they have completed Level II clinical experience; Level I observation-only experience does not qualify.
Must be able to provide skilled occupational therapy services in a Long Term Care/rehabilitation setting.
