Registered Nurse (RN) – Case Manager

Location: Pittsfield, MA

Schedule: 7:00 AM – 3:30 PM
Hours: 40 hours/week – 5 × 8-hour shifts

Position Overview

Berkshire Medical Center is seeking an experienced Registered Nurse Case Manager to manage patient care needs, coordinate resources, and support quality and cost-effective outcomes throughout the continuum of care.

Responsibilities
Manage and coordinate healthcare needs for patients throughout their hospital stay.
Perform admission, concurrent, and discharge assessments.
Evaluate appropriate level of care and utilization of healthcare resources.
Develop and coordinate transition-of-care and discharge plans.
Coordinate post-hospital services and resources.
Collaborate with physicians, multidisciplinary teams, patients, families, and insurance organizations.
Monitor patient progress and resource utilization.
Support quality and performance improvement initiatives.
Maintain accurate documentation and confidentiality.
Analyze and interpret clinical, financial, and utilization data as needed.
Required Qualifications
Active Massachusetts RN license in hand at submission.
BLS from AHA or American Red Cross.
3+ years of clinical experience in acute care, including areas such as internal medicine, surgery, orthopedics, cardiology, oncology, or mother/child.
Candidates with 3+ years of experience in Rehab, Home Health, or Long-Term Care may also be considered.
Strong case management, assessment, communication, negotiation, and organizational skills.
Knowledge of Medicare, Medicaid, commercial payers, and reimbursement.
Ability to work independently and collaborate with physicians and multidisciplinary teams.
Knowledge of local community resources.
Strong computer and data management skills.
Own transportation/vehicle required.
