Junxion Med Staffing is looking for a Case manager RN to take an assignment in Lone Tree, Colorado.

**Must have:** - Knowledge of discharge planning - Patient assessment - Patient family conferences - Experience working with physicians on safe discharge plans - Experience with skilled nursing facilities, home health, hospice, and long-term acute care hospitals (LTACH) **Preferred or nice to have:** - Experience with Meditech EMR system **Best Personality Fit:** - Detail-oriented - Enjoys working with a multi-team approach - Can handle stress well - Patient-focused
