Licenses and Certifications: License/Certificate Verification is required. Training/Health Requirements: COVID-19 Vaccine, MMR, Fit Test, Hepatitis B, Influenza Vaccine, 2 Step PPD OR IGRA (Quantiferon/Tspot), Varicella, Physical/Health Statement, and Trinity 10 Panel Drug Screen are required for onboarding. Professional Documentation: Trinity Profile is needed for submission, while Trinity Conf. of Patient & Pty. Info. & System Security Agmt., Trinity Conflict Of Interest Disclosure Form, Trinity Criminal Background Check, Trinity Education Verification, Trinity EPLS, Trinity GSA/OIG, Trinity Integrity and Audit Services Form, Trinity Joint Commission Testing, Trinity NSO, Trinity System Access Form, Trinity Code of Conduct Form, Trinity I9, and Trinity Specialty Specific Competency Testing are required for onboarding. Skills and Testing: Trinity Skills Checklist and Trinity Specialty Specific Competency Testing are required for onboarding. Vision Requirements: Trinity Color Vision for onboarding.
