• Certifications: BLS Provider, Cardiographic Tech credentialed from CCI, Radiologic Technologist specializing in Radiography credentialed from ARRT, Radiologic Technologist issued by Illinois Emergency Mgmt Agency

**Licensure / Certification / Registration:** 1. BLS Provider from the American Heart Association or American Red Cross required. 2. Cardiographic Tech credentialed from the Cardiovascular Credentialing International (CCI) obtained prior to hire date or job transfer date. 3. Radiologic Technologist specializing in Radiography credentialed from the American Registry of Radiologic Technologists (ARRT) obtained prior to hire date or job transfer date. Any specialty accepted. 4. Radiologic Technologist issued by Radiology Tech licensure from the Illinois Emergency Management Agency required in combination with American Registry of Radiologic Technologists. 5. Registered Cardiovascular Invasive Specialist credentialed from the Cardiovascular Credentialing International (CCI) preferred. **Education and Experience:** - High School diploma equivalency with 1-2 years of cumulative experience OR - Associate's degree/Technical degree OR - 4 years of applicable cumulative job specific experience required. **Health and Safety Requirements:** - COVID-19 Vaccine - MMR - Fit Test - Hepatitis B - Influenza Vaccine - TB Test - Varicella - Tdap - Physical/Health Statement **Professional Tests and Attestations:** - Prime Core Competencies Exam - Prime Skills Checklist - Onboarding Education Attestation **Additional Onboarding Requirements:** - Driver's License/State ID - Prime Drug Screening (minimum 10-panel test) - Prime Color-blind test - Prime Criminal Background Check - Prime Badge Photo - Prime Form I-9 (Employment eligibility verification) - Essentials Onboarding Packet - Professional: Prime Agency Attestation - Name Verification Affidavit (Optional)
