The job description outlines several requirements and preferences related to experience, certifications, licenses, and qualifications: 1. **Licensure and Certifications**: - License/Certificate Verification is required for submission. - Driver's License/State ID is required for submission. - Ascension Medical Imaging Contract Worker Onboarding Education Attestation is required for onboarding. - Health-related certifications such as MMR, Hepatitis B, 2 Step PPD OR IGRA, Varicella, and Tdap are required for onboarding. 2. **Professional Requirements**: - Ascension Profile and 2 Professional References are required for submission. - Ascension Criminal Background Check Results, EPLS, Skills Checklist, 12 Panel Drug Screen, Education Verification, Confirmation of Assignment, Employment Verification, and Badge Photo are required for onboarding. 3. **Medical and Health History**: - Health Statement is required for onboarding. - Fit Test, Health Statement, and various vaccinations are noted as required for onboarding, including Influenza Vaccine, which is optional. 4. **Additional Notes**: - The position requires understanding of Ascension Core Competencies and Skills Checklist, which are required for onboarding. - Optional onboarding items include Professional:Hiring Manager Evaluation and Name Change Document. - Certifications and licenses need to be verified as part of the process. These components must be completed for submission and onboarding to meet the job's requirements.
