The job position requires several credentials and qualifications for onboarding: 1. **Licensure and Certifications:** - License/Certificate Verification. 2. **Health and Safety Requirements:** - Fit Test. - Trinity 10 Panel Drug Screen. - COVID-19 Vaccine. - MMR (Measles, Mumps, Rubella). - Hepatitis B. - 2 Step PPD OR IGRA (Quantiferon/Tspot). - Varicella. - Physical/Health Statement. - Optional: Influenza Vaccine. 3. **Professional and Compliance Requirements:** - Trinity Code of Conduct Form. - Trinity I9. - Confirmation of Patient & System Security Agreement. - Integrity and Audit Services Form. - Non-Employee Upload Form. - Trinity Conflict Of Interest Disclosure Form. - Trinity Criminal Background Check. - Education Verification. - Trinity EPLS (Excluded Party List System). - Trinity GSA/OIG (General Services Administration/Office of Inspector General). - Joint Commission Testing. - Trinity NSO (Nursing Staff Organization). - System Access Form. - Child and Adult Dependent Abuse Training. - Trinity IA Sing Report. - Trinity OFAC (Office of Foreign Assets Control). 4. **Testing:** - Trinity Specialty Specific Competency Testing. - Trinity Skills Checklist. - Trinity PowerPoint Genesis (possibly indicative of training or testing via a PowerPoint format). These requirements are necessary for either submission or onboarding, with some being optional.
