The job description outlines several required and optional health history checks, certifications, licensures, and professional forms needed for onboarding. Key credentials include: 1. **COVID-19 Vaccine**: Required for onboarding. 2. **Trinity 10 Panel Drug Screen**: Required for onboarding. 3. **Licensure/Certificate Verification**: Required for onboarding; necessary for both certifications and licensure. 4. **Professional Documents**: Includes Trinity Code of Conduct Form, I9 form, Confidentiality Agreement, Conflict of Interest Disclosure, Criminal Background Check, Education Verification, EPLS, GSA/OIG checks, Integrity and Audit Services Form, Joint Commission Testing, NSO, and System Access Form— all required for onboarding. 5. **Health History**: MMR, Varicella, Physical/Health Statement, Color Vision, Fit Test, Hepatitis B, and 2 Step PPD OR IGRA are required for onboarding. 6. **Skills and Competency Testing**: Trinity Specialty Specific Competency Testing and Skills Checklist are required for onboarding. The only optional requirement is the Influenza Vaccine for onboarding. These credentials and verifications are mandatory upon the applicant's onboarding process to fulfill compliance and competency standards.
