RN Case Manager – Travel Contract | Jackson, MS | 13 Weeks | Days

Alto Health Care Staffing is seeking an experienced RN Case Manager for a 13-week travel assignment in Jackson, MS. This is a 40 hr/week day shift (Mon–Fri 8:00a–4:30p) role supporting inpatient units at a Level 1 academic medical center with a collaborative case management and social work team.

Assignment Details

• 13 weeks | 40 hours per week

• Day shifts: Monday–Friday, 8:00 AM – 4:30 PM

• No weekends, holidays rotated as needed

• Caseload: 15–20 patients/day

• Travelers must live 50 miles away (locals accepted at local rate)

Responsibilities

• Acute inpatient case management and discharge planning

• Coordinate patient care transitions across inpatient units

• Collaborate with a large interdisciplinary team including RN Case Managers, Social Workers, Pharmacy, and Respiratory Therapy

• Work with regional partner facilities for complex discharge placements

Requirements

• ​RN (BSN preferred, ADN considered)

• Minimum 3 years Acute Inpatient Case Management experience

• At least 1 year in a large hospital (500 beds / Level 1 or academic medical center)

• Recent acute inpatient CM experience within the last 2 years

• Epic EMR experience required

• ​BLS (AHA) required | CCM preferred
• ​Strong discharge planning and care coordination experience

• Covers inpatient units across the hospital

• Works alongside a team of 15 full-time and 17 part-time social workers

• 2-week department orientation (minimal training expected afterward)

• Flu vaccine required (medical exemption only)

• Fingerprinting required – remote (allow 10 days) or in person prior to start
