Hours per Week: 40 (Full-Time)
Shift: 8:00 AM – 4:30 PM (8-Hour Day Shifts)
Weekend Rotation: Once every 6 weeks
Estimated Weekly Salary: $2,330 - $2,500 per week

Registered Nurse Qualifications

Active Registered Nurse (RN) license in Massachusetts
At least 1 year of certified home health care experience
Minimum 1 year of case management experience in a clinical setting
Ability to travel to patient homes throughout Newton and the Boston area
Strong patient assessment, care planning, and clinical documentation skills
Proficiency with electronic medical records (EMR/EHR)
Excellent communication and interdisciplinary collaboration skills

Home Health RN Responsibilities

Assess patient conditions and develop individualized homecare plans
Provide patient education and support to families in the home setting
Deliver high-quality patient care in accordance with healthcare policies and community health standards
Perform admission processes and manage patient cases effectively
Maintain accurate and timely electronic documentation
Collaborate with the interdisciplinary healthcare team
Participate in scheduled weekend rotations as required
