Lee Health Heart Institute is currently seeking an experienced Registered Nurse to join our Structural Heart Team as a Nurse Navigator. This is a rewarding opportunity to support patients through their cardiac journey while working alongside a collaborative, multidisciplinary team.
About the Role:
The Nurse Navigator serves as a consistent point of contact for patients from diagnosis through treatment and follow-up. Youll work closely with Structural Interventional Cardiologists, CT Surgeons, the Structural Heart Medical Director, and other members of the Lee Health Heart Institute.
Key Responsibilities:
Coordinate care throughout the continuumfrom diagnosis to discharge
Act as a liaison among patients, families, care teams, and referring providers
Deliver education and consultation both in person and over the phone
Connect patients with community resources and support
Advocate for patients, removing barriers to timely, effective care
Qualifications:
- Bachelors in nursing required, Masters preferred.
- Active RN license in Florida (or eligibility to obtain)
- Minimum of 3 years of cardiac nursing experience
- Excellent communication, critical thinking, and organizational skills
- Experience in structural heart, interventional cardiology, or cardiovascular surgery preferred
