Department: LPN Home Health - South Team (Fort Myers, Estero
Work Type:Full Time (80 hours bi-weekly)
Shift: Days, 8:00 AM to 4:30 PM, Monday - Friday Work Week
Minimum to Midpoint Pay Rate:$22.34 - $29.04 / hour

Key Responsibilities:
- Conduct comprehensive assessments, including OASIS, to establish tailored care plans.
- Collaborate with a multidisciplinary team to ensure seamless and efficient patient care.
- Engage in meaningful education and counseling for patients, caregivers, and families.
- Implement specialized nursing treatments and preventive measure.
- Document progress and coordinate services, ensuring all team members are informed of patient updates.
- Supervise LPNs and home health aides, fostering a supportive environment for all staff.
- Participate in professional development and quality improvement initiatives.

Performs and documents selected tasks such as the administration of treatments and medications in delivery of patient care under the guidance and discretion of the registered nurse. Assist the Register Nurse in collecting health data according to established policies. Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. Supports the organization vision and mission. Uses knowledge of patients age and cultural diversity into the provision of patient care. Contributes to the provision of quality nursing care through excellence in customer service and performance improvement techniques that demonstrate positive outcomes in patient care.
Requirements
Education: Graduate of an accredited vocational school of nursing required.
Experience: None required. Qualified individuals will successfully complete the competency assessment.
Certification: Proof of American Heart Association Healthcare Provider Basic Life Support (BLS), current for a minimum of 90 days after hire date. 30-hour IV Course required.
License: Currently licensed as an LPN in the State of Florida.
Other: Communication and interpersonal skills, basic computer skills, and customer service skills required.
