Description
Location:HealthPark Care and Rehabilitation Center -16131 Roserush CtFort Myers FL 33908
Department:Nursing - Skilled Nursing Facility (SNF): (112 bed facility) One Level Building
Work Type: Full Time (80 hours bi-weekly)
Shift: Days, 7:00:00 AM to 3:30:00 PM (Monday - Friday) Some Holiday Rotation
Minimum to Midpoint Pay Rate:$23.13 - $28.92 / hour
Summary
This position is responsible for monitoring the nursing care needs of residents and gathering data for the Minimum Data Set (MDS).
The coordinator communicates with the interdisciplinary team and collects information necessary for reimbursement and quality reporting related to Medicare, managed care, and long-term care patients.
It is essential to ensure the accuracy of MDS and care plans for both Medicare and non-Medicare patients. Additionally, they will coordinate the updating of care plans as needed to reflect the level of care being provided to each resident.

Requirements
Education:Graduate of an accredited school of nursing required.
Experience:1-year MDS experience preferred.
Certification: RAC-CT required within one year of hire.
License: FloridaLicensed Practical Nurse required.
Other: Excellent communication and customer service skills required.
