Physical Therapist - Acute Care Hospital

Location: Dubois, Pennsylvania
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

• Provide physical therapy services in an inpatient acute care setting.
• Typical shift: Monday to Friday, 7:30 AM to 4:00 PM (40 hours/week).
• Flexibility required to potentially stay longer on Mondays and Tuesdays, with the possibility of shortening another day in the week to maintain 40 hours (approval required).

Required Qualifications

• Valid Physical Therapist license in Pennsylvania
• Minimum 1 year of Physical Therapist experience within the last year
• Basic Life Support (BLS) certification required

Compliance and Documentation Requirements

• Government-issued photo ID required.
• Background checks including:

  • 7-year criminal history search (including SS trace, county searches, nationwide, sex offender, and state-specific checks).
  • EPLS/GSA/SAM and HHS/OIG checks.
• Education verification (transcripts, diplomas, or third-party verifications).
• Pennsylvania Child Abuse Search (Childline) within 1 year of start.
• Pennsylvania FBI fingerprinting required within 12 months of start (must be completed in PA).
• Physical exam within 1 year of start.
• Negative TB test (PPD, Q Gold, or TSpot) within 6 months of start or chest X-ray within 5 years.
• Immunization or positive titer documentation required for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Tdap vaccination within the past 10 years (declination accepted with form).
• COVID-19 vaccination or facility-specific declination required.
• Seasonal flu vaccination or declination required during flu season (Nov-April); mask required if declining.
• Negative 10-panel drug screen within 30 days of start.
• Alcohol breathalyzer test not required for this location.
• Two professional references within the last 3 years.
• Annual skills checklist and competency exams as applicable.

Additional Notes

• Photo (passport style, no filters) required for badge issuance.
• Completion of facility-specific orientation packet, medical surveillance checklist, and vaccine declination forms (if applicable) required prior to start.
• Management of Assaultive Behavior (MAB) or Crisis Prevention Institute (CPI) certification may be required for certain units but not specified for this role.

Shift Details

• Day shift, Monday through Friday, 8 hours per day.
• Flexibility in shift hours to meet 40-hour workweek requirements.
