Job Overview

Required Qualifications

• Valid Pediatric Speech Language Pathologist license in Ohio
• Pediatric Speech Language Pathologist (1 year of experience)
• Pediatric Speech Language Pathologist experience within the last year
• BLS - Basic Life Support issued by American Heart Association or American Red Cross or United States Department of Defense

Compliance / Onboarding

• Government-issued photo ID required
• Proof of E-Verify or attestation required
• Background check required before start and updated for breaks in service of 2 or more months
• FBI and BCI fingerprinting required within 12 months of start
• Primary source license verification required
• NURSYS verification required as applicable
• Physical exam required within 12 months of start
• Negative TB test or chest X-ray required within 1 year of start
• Drug screen required within the last 12 months
• Two clinical references required
• Unit-specific skills checklist required and updated annually
• Competency exam required and updated annually
• HealthStream modules required on orientation

Health Requirements

• Tdap documentation required
• Measles, mumps, rubella, varicella, and hepatitis B immunity documentation required
• Seasonal flu vaccination or approved exemption required
