Care Mgr

Overview

• Travel assignment in Longview, Texas
• Full-time
• Contract length: 13 weeks
• Schedule: 5 days, 8 hours per day
• High pace environment
• Daily caseload: 20–25 patients
• May be asked to arrive at 7:30 AM and stay until all cases are finished for the day

Required Qualifications

• Valid RN license in Texas
• Case Management (2 years of experience)
• Case Management experience within the last year
• Associate of Science in Nursing / Associate Degree in Nursing
• Experience with MCG criteria
• Experience with utilization review (InterQual)
• Experience with Epic

Preferred

• Acute Hospital Management
• One year of acute care experience
• Certification in Case Management
• BLS
• BSN

Role Details

• Care coordination and discharge planning are responsibilities of this role
• Works with patients, families, physicians, and multidisciplinary team members to support patient progression through the continuum of care
• Focuses on patient flow, throughput, continuity of care, safe transitions, patient satisfaction, patient safety, and length of stay management
• May assist with special projects
• May serve as a preceptor or orienter to new associates
