Job Description

• Role involves discharge planning and case management in an acute care or rehab setting.
• Patient ratio: 1 Case Manager to 16 patients.
• Common diagnoses include stroke, brain injury, spinal cord injury, general medical, and orthopedic injury.
• Work closely with a multi-disciplinary team, managing conflict and emotional situations involving patients and families.
• Facility uses Cerner EHR, transitioning to EPIC in June.

Required Qualifications

• Valid Registered Nurse (RN) license in Michigan.
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
• Minimum 1 year of case management experience within the last year.
• Minimum 1 year of discharge planning experience in acute care or rehabilitation settings.
• Experience working in both acute care and rehabilitation settings.
• Strong communication and organizational skills.
• Ability to manage conflict and emotional situations effectively.
• Required certifications appropriate for RN role.

• No weekend or call requirements.
• No float requirements.
