Registered Nurse - Case Management (Travel)
Location: Southfield, Michigan
Contract Length: 13 weeks
Shift: Days
Hours per Week: 32 (40 guaranteed weekly hours noted in unit profile)
Unit: Rehab Case Management

Job Description
- Role involves discharge planning and case management in an acute care or rehab setting.
- Patient ratio: 1 Case Manager to 16 patients.
- Common diagnoses include stroke, brain injury, spinal cord injury, general medical, and orthopedic injury.
- Work closely with a multi-disciplinary team, managing conflict and emotional situations involving patients and families.
- Facility uses Cerner EHR, transitioning to EPIC in June.
Required Qualifications
- Valid Registered Nurse (RN) license in Michigan.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Minimum 1 year of case management experience within the last year.
- Minimum 1 year of discharge planning experience in acute care or rehabilitation settings.
- Experience working in both acute care and rehabilitation settings.
- Strong communication and organizational skills.
- Ability to manage conflict and emotional situations effectively.
- Required certifications appropriate for RN role.
Additional Information
- No weekend or call requirements.
- No float requirements.
- Dress code: Business casual.
- Report time: 8:30 AM.
- Parking available in visitor lot.
- Personality fit: Must work well with diverse personalities and support patients/families through difficult times.
