Job Description

• Work in an inpatient rehabilitation hospital providing services for patients recovering from strokes, brain or spinal cord injuries, amputations, complex orthopedic injuries, and other conditions.
• Facility features include a low stimulation and secured unit with monitored rooms, specialized beds, patient lifts, dedicated therapy gym and dining area, high-tech therapy devices, therapy car, special apartments for practicing daily living tasks, private family-friendly rooms, and an outdoor healing garden and walking trail.
• Assist with discharge planning, utilization review, family and team conferences, and other patient needs to facilitate safe discharge.

Schedule & Work Details

• Shift: 8:00 AM – 4:00 PM
• Weekly Guaranteed Hours: 32
• Weekend Hours: None
• Call Requirements: None

Unit Information

• Number of Beds: 36
• Patient Population: Adults 18+ with cardiac, orthopedic, neurological, debility, and general inpatient rehabilitation diagnoses (e.g., CVA, TBI, amputation).
• Electronic Health Records (EHR): Pinpoint, Rehab Connect

Required Qualifications

• License: Current Registered Nurse (RN) licensure required.
• Education: Bachelor’s or Master’s degree in Social Work or Nursing preferred.
• Certifications: Certification in Case Management or Rehabilitation preferred (e.g., CCM, ARN, ACM, RN-BC).
• Experience: Minimum 2 years of social work or case management experience in an inpatient setting highly preferred; acute or rehabilitation hospital experience preferred. Previous leadership or supervisory experience strongly preferred.
• Skills: Basic computer knowledge, effective verbal and written communication in English, cooperative team player.

Additional Requirements

• Compliance with facility’s Influenza Vaccination Policy or approved exemption with masking during flu season (Nov 1 – Mar 31).
• Dress code: Black scrubs or business casual.
