Registered Nurse - Case Management (Inpatient Rehabilitation)

Location: Los Gatos, California
Employment Type: Travel
Contract Length: 13 weeks

Position Overview

• Role: Clinical Liaison (RN)
• Specialty: Inpatient Rehabilitation
• Setting: Acute Rehab Unit, separate building on hospital campus
• Unit Size: 30 beds
• Patient Population: Adult and Geriatric
• Patient Ratio: 5:1
• Shift: Monday through Friday, with 1 weekend per month (4 hours Saturday + 4 hours Sunday; flex off following Thursday)
• Weekly Hours: 40 guaranteed
• Call Requirements: None
• Float Requirements: May float to cover different facilities; some local travel expected

Required Qualifications

• Valid Registered Nurse (RN) license in California
• Associate of Science in Nursing / Associate Degree in Nursing
• Minimum 1 year of case management experience, including experience within the last year
• Experience in acute hospital and post-acute care case management settings
• Knowledge of care coordination, case management, and business development/marketing

Common Diagnoses/Treatments

• Stroke/CVA
• Brain Injury
• Spinal Cord Injury

Additional Notes

• No required certifications beyond RN license
• No call responsibilities
• Some local travel to neighboring hospitals may be required due to float duties
