Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates assigned patients and develops, implements, and modifies treatment plans according to patient needs, goals, and physician direction. This role is responsible for delegating and directing discipline-specific therapy services for all assigned patients and ensures design of functional treatment programs that enable patients to effectively return to community activities. The therapist serves as an educator for patients, families, students, coworkers, and engages in ongoing professional development.

Responsibilities

• Complete comprehensive evaluations of assigned patients and utilize clinical reasoning to create appropriate treatment plans and goals.
• Document medical records clearly, concisely, accurately, and timely, adhering to state, federal, and regulatory guidelines.
• Ensure that care is provided under signed, valid physician orders and conforms to insurance approval if necessary.
• Participate in student programs as assigned, offering supervision and developing evaluations.
• Create and execute an individualized professional development plan for ongoing growth.
• Maintain professional licensure in good standing and fulfill all state-mandated continuing education requirements.
• Actively engage in performance improvement initiatives to enhance the quality of services provided.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR Certification.
• Active licensure in the state where services will be performed.
• Previous experience with the agency is highly desired.
• Prior experience with Select Medical is a plus.

Schedule

This is a 5x8.0 shift position, starting ASAP.
