Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patient’s needs, goals, and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for those assigned patients, ensuring the design of functional treatment programs that allow for effective return to community activities.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Develop appropriate treatment plans and goals focused on progressing patients towards discharge within the established length of stay.
• Complete medical record documentation accurately, clearly, and in a timely manner, including charge entry, goal writing, and letters of medical necessity.
• Ensure care is provided under signed, valid physician orders.
• Participate in student programs as assigned, offering a positive learning environment.
• Create an individualized professional development plan and pursue ongoing professional growth.
• Adhere to the discipline-specific State Practice Act and maintain professional licensure and continuing education requirements.
• Engage in performance improvement initiatives to enhance quality of services provided.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• Two years of outpatient clinical experience.
• Current CPR certification.
• Active and applicable license for the state where work will be performed.

Schedule

This position operates on a 5x8.0 schedule with two evenings required, totaling 40 hours per week.

Assignment Duration

This is a 13-week contract position.
