Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist is responsible for evaluating assigned patients and developing, implementing, and modifying treatment plans in accordance with patients' needs, goals, and physician direction. This role ensures the design of functional treatment programs that allow for effective return to community activities and serves as an educator for patients, families, students, and coworkers. The therapist also participates in professional duties related to patient care and discipline-specific therapy services.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals.
• Document care accurately and timely, ensuring compliance with all guidelines.
• Provide care under valid physician orders and within insurance approvals.
• Participate in student program as assigned, providing supervision and evaluations.
• Create and pursue an individualized professional development plan.
• Adhere to discipline-specific State Practice Act and maintain licensure in good standing.
• Participate in the organization’s performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure.
• Previous experience with the agency is highly desired.
