Schedule

Days: 8am-5:30pm (Schedule can be flexible based on patient's availability.)
Weekends: One weekend required

Job Description

The Physical Therapist's primary goal is to restore function to prevent or minimize disability following disease or injury and to help clients reach their maximum performance/function within their capabilities. They will evaluate and treat patients by utilizing medically prescribed physical therapy programs. Responsibilities may also include supervising Physical Therapy Assistants, making initial admission/evaluation visits, and notifying the assigned Clinical Team Manager of OASIS scores within established timelines. The ideal candidate will document observations, clinical findings, problems, skilled interventions/treatments, goals, and discharge plans, in alignment with the goals and mission of the organization.

Responsibilities

• Evaluate and treat patients using medically prescribed physical therapy programs.
• Document observations, clinical findings, problems, skilled interventions/treatments, goals, and discharge plans.
• Supervise Physical Therapy Assistants and make initial admission/evaluation visits.
• Notify assigned Clinical Team Manager of OASIS scores within established timelines.
• Perform other duties as assigned consistent with skills and training.

Required Experience

One year of Home Health experience is required.
