Day shift, 5x8-hour schedule (08:00 AM - 04:30 PM). This facility cannot accommodate 4x10s or 3x12s.

Job Description

We are currently seeking a dedicated Physical Therapist to join our team in a Long Term Care / Skilled Nursing Facility in Seattle, WA. The ideal candidate will provide rehabilitation services and support to patients in a skilled nursing setting, contributing to their recovery and overall well-being.

Responsibilities

• Conduct evaluations and provide treatment plans for patients requiring physical therapy.
• Administer therapy interventions in alignment with established treatment goals.
• Document patient progress and update treatment plans as necessary.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Educate patients and caregivers on exercises, techniques, and recovery processes.

Required Experience / Certifications / Licensure

• A minimum of 2 years of experience in Skilled Long Term Care or Transitional Care is required.
• Current Physical Therapist license in the state of Washington.
• BLS certification is required.
• Experience with central line blood draws and dressing changes, IV pumps, and starting IVs is preferred.
• Previous charge experience is preferred.
