Job Description
We are seeking a skilled Home Health Occupational Therapist with at least one year of experience to provide exceptional therapy services to patients primarily in their homes. The role involves evaluating patients, developing treatment plans, and delivering targeted therapy interventions.
Responsibilities
- Conduct patient assessments and develop individualized treatment plans.
- Implement therapy interventions in accordance with established protocols.
- Monitor patient progress and adjust treatment plans as needed.
- Provide education and support to patients and families on health and wellness topics.
- Document therapy services and patient outcomes according to agency policies.
Required Experience / Certifications / Licensure
- Minimum of 1 year of skilled home health experience.
- Current Occupational Therapist license in Oregon.
- BCLS certification required.
- OASIS experience is mandatory.
- Reliable transportation is required for patient visits.
