Case Management RN - Ortho Care Coordinator

Assignment Duration

This is a 13-week contract position.

Schedule

Monday through Friday, 8:00 AM to 4:30 PM or 8:30 AM to 5:00 PM. No weekends required.

Job Description

We are seeking a skilled Case Management RN specializing as an Ortho Care Coordinator. The ideal candidate will have experience in case management, discharge planning, and patient education post-surgery. Expertise in Parachute and Careport software related to orthopedic care is preferred.

Responsibilities

• Conduct case management duties including discharge planning and patient navigation.
• Educate patients on post-surgery care and recovery plans.
• Collaborate with clinical teams to ensure comprehensive care management.
• Utilize software tools effectively to support orthopedic patient coordination.

Required Experience / Certifications / Licensure

• Minimum 3-5 years of relevant clinical experience in medical-surgical, ICU, or ER settings preferred.
• Experience in managed care, conducting utilization review and discharge planning is preferred.
• Current New York State RN license is required.
• BLS certification is required; ACLS certification is preferred.
• HC-PRI Certified and PRI screen preferred. CCM certification preferred.
