Physical Therapist

Job Description

The Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans according to the patients' needs and physician direction. Responsible for delegating and directing all discipline-specific therapy services for assigned patients. Ensures the design of functional treatment programs that facilitate effective returns to community activities. Also serves as an educator for patients, families, students, coworkers, and self, and performs other professional duties associated with patient care and the design of program delivery.

Responsibilities

• Completes comprehensive evaluations of assigned patients.
• Utilizes clinical reasoning to develop appropriate treatment plans and goals focused on patient discharge within an established length of stay.
• Collaborates with the interdisciplinary team to address barriers to discharge, including timely equipment trials, patient and family training, and continuing services planning.
• Manages individual, concurrent, and group treatment sessions based on patient care needs.
• Documents medical records accurately and in a timely manner, including daily documentation, goal writing, and discharge documentation.
• Ensures care provision under signed, valid physician orders and within insurance approval.
• Participates in student programs, providing supervision and completing evaluations.
• Engages in professional development through continuing education and self-study.
• Participates in patient and professional advocacy efforts, including health fairs and community events.
• Maintains professional licensure in good standing and adheres to discipline specific guidelines and codes of conduct.

Required Experience / Certifications / Licensure

• BLS Certification Required Prior to First Shift
• Must hold a valid, current State license
• Minimum of 2 years of experience in a related role

Schedule

This position involves a day shift with a 5x8.0 schedule.
