Job Description
We are seeking a dedicated and skilled professional to join our team in the clinical setting. The ideal candidate will be responsible for providing high-quality care to patients while collaborating with a multidisciplinary team.
Responsibilities
- Administer patient assessments and develop care plans.
- Coordinate with healthcare providers to implement treatment plans.
- Monitor patients' progress and adjust care as necessary.
- Provide education and support to patients and their families.
Required Experience
- Minimum of 2 years of relevant clinical experience.
- Experience in a hospital or healthcare setting preferred.
Certifications / Licensure
- Valid state license in the appropriate profession.
- Required certifications relevant to the specialty area.
