JOB DESCRIPTION:
RN - Continuing Care

About Facility:

Skilled Nursing Facility
625 Bethany Rd, Albemarle, NC 28001
Radius rule: Traveler's permanent address must be 50 miles outside of home unit

Department Information:

Beds: 90
Minimum Years of Experience required? 18 months in SNF/LTC Rehab facility
Will you accept a first-time traveler? Not preferred

Patient Types/Common Diagnoses:

Short-term and Long-term services offered to patients recovering from an illness, injury or accident
Stroke, Amputation, Neurological Disorders, s/p Joint Replacement, etc
No Memory Care

Patient Ratios:

RN: 1:18-20 Days; 1:26 Nights
LPN: 1:18-20 Days; 1:26 Nights
CNA: 1:13 Days; 1:15 Nights

Required License:

National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements
RN: BLS
LPN: BLS
CNA: BLS

Skills required:

Previous Long-Term Care exp to include: Trach management (refresher provided during orientation), foley care, wound experience, dressing changes, IV starts/management, tube feedings

Support within the Department:

Nursing Leadership
Admin Nurse will do any admission – admissions will be approx. 1-2 per day
HCT
Therapies
Case Management

Technology/Equipment:

EMR: EPIC
IV Pump: Alaris
Med dispensing: Omnicell

Floating:

Within Continuing Care and scope of practice
Will be required to float per region needs – will require floating to all facilities listed below, as needed, within scope of practice – DECLINE if not agreeable
Rehab Region - Cleveland Pines, Huntersville Oaks, Jesse Helms, Sardis Oaks, and Stanly Manor
If floating mid-shift, Traveler will be paid for the time traveling between facilities (will remain on the clock)

2 days departmental
