JOB DESCRIPTION
RN Hospice Case Manager

RN Hospice Case Manager- Sheboygan County team. 13-15 caseload seeing patients in home/ALF/SNF. Previous admission experince and EPIC remote required

● About Facility:
o Address: Servicing Sheboygan and Manitowoc Counties
● General Information:
o Will you accept a traveler with 1-year experience? 2 years of Hospice experience needed
o Will you accept a first-time traveler? Yes, if they have Hospice and EPIC Remote Client experience
● Patient Types:
o Adults only – End of Life patients
● Patient Ratios:
o 1:1
● Required Certifications:
o BLS
o Valid DL and Insurance
● Skills required:
o End of life care/planning, wound assessments, care coordination, pain/symptom management, triage
● Support on the Unit:
o RNs
o Case Managers
o Manager on call
● Technology/Equipment:
o EMR: EPIC Remote Client – Required
o IV pumps: Sapphire
● Floating: If yes, where?
o Anywhere within the counties
● Orientation (how many shifts): up to 1 week, based on comfort level
● Scheduling:
o Weekend rotation: One weekend a month
o Call? One call shift a month
o Holiday Expectations: No holidays
o Approval of time off: Can we approve time off on your behalf, or would you like all requests sent to you? Manager to approve all RTO
o Shift times: M-F 0800-1630

o Scrub Color: any color or business casual
