Description
RN – Hospice Case Manager
Location: Burlington, WI (Onsite)
Shift: 8-hour day shift (Monday–Friday, approx. 8:00 AM – 4:30 PM)
Weekly Hours: 40
Weekend/Holiday/On-Call: Not required
Floating: Within the community to patient homes
Patient Population
- End-of-life / hospice patients
- Home hospice and facility-based hospice care
- GIP (General Inpatient) hospice cases in hospital settings
Responsibilities
- Provide direct RN case management for hospice patients
- Deliver skilled nursing care in patient homes and facilities
- Coordinate interdisciplinary hospice care plans
- Utilize EMR for documentation and care coordination
- Manage assigned patient caseload efficiently and safely
- Collaborate with hospice care team to ensure quality end-of-life care
Caseload & Staffing
- RN Case Managers: 7 total RNCMs in team
- Caseload: approximately 14–16 patients per RN
- CNA support: 6 CNAs on team
- Average census: ~105 patients/day across program
Required Qualifications
- Active RN license
- Minimum 2 years of nursing experience (hospice/home health preferred)
- Current BLS certification
- Valid driver’s license and auto insurance (required for home visits)
- Strong clinical assessment and case management skills
Skills & Experience
- Hospice or home health nursing experience preferred
- Strong independent clinical judgment
- Experience with end-of-life care
- Comfortable with community-based patient care
Technology & Equipment
- EMR: Epic (Rover enabled)
- IV pumps (as needed)
- Medication dispensing systems
- Standard home hospice equipment and supplies
