JOB DESCRIPTION
CH Hosp Milwaukee North 12 - Hospice

● General Information:
· Beds: N/A
· Will you accept a traveler with 1-year experience? 2 years of Hospice experience needed
· Will you accept a first-time traveler? Yes, if they have Hospice and EPIC Remote Client experience
● Patient Types:
· Adults only – End of Life patients
● Patient Ratios:
· 1:1
● Required Certifications:
· BLS
· Valid DL and insurance
● Skills required:
· End of life care/planning, wound assessments, care coordination, pain/symptom management, triage
● Support on the Unit:
· RNs
· Resource Nurse
· Case Managers
● Technology/Equipment:
· EMR: EPIC Remote Client – experience preferred
· IV pumps: Sapphire
● Floating: If yes, where?
· Anywhere within the counties
● Orientation (how many shifts): up to 1 week, based on comfort level
● Scheduling:
· Weekend rotation: No weekends
· Call? No call
· Holiday Expectations: No holidays
· Approval of time off: Can we approve time off on your behalf, or would you like all requests sent to you? Manager to approve all RTO
· Shift times: M-F 0800-1630; (4) 10 hour shifts may also be available
