Number of Staff – Team of CM/SW to manage load

Day Shift – Friday-Monday 8-4:30
Day Shift – Monday –Friday 8-4:30
Night Shift – N/A
Patient Ratios – SW: 1:40 RN: 1:20
Type of equipment – Mobile and Computer
EMR – Meditech/Midas/ CarePort
Typical hiring profile
Skill Set Most recruited for: (Travel RNs, local CNAs and sitters)
Must have: candidates without these skills will not be considered for the role.
Minimum 1-year experience as a Social Worker preferrable in an acute care setting
Active CO licensure
Preferred or nice to have: candidates with these skills will be considered first.
Discharge planning
ED experience
Palliative Care Knowledge and GOC Conversation
Behavioral Health experience with substance uses assessments
