Facility: Northside Hospital Forsyth – Outpatient Rehab Services
Address: 2000 Howard Farm Dr #120, Cumming, GA 30041
Schedule: M–F, 8:00 AM–6:00 PM
Local Rule: No local candidates within 50 miles
Experience: 2 years in specialty; no new grads
Required: AHA BLS, GA PT License
Education: Bachelor's degree or higher from an accredited PT program
Preferred/Required Specialty:
- Pelvic PT: Herman & Wallace Level 1 or APTA Pelvic Health certification
- OR Orthopedic PT: Strong post-op, spine, lower-extremity & sports experience
Holiday: Must work 2 of 3 major holidays 1 minor holiday
