Monday to Friday, 8:00 AM to 4:30 PM
40 hours per week

Job Description

Coordinate management of patient care to ensure optimal resource utilization, service delivery, and compliance with external review agencies.
Provide comprehensive assessment, care planning, implementation, and evaluation of individual patient needs.
Enhance quality of patient management and satisfaction by promoting continuity of care and cost-effectiveness.
Integrate case management, utilization review, and discharge planning functions.
Serve as a patient advocate addressing holistic patient needs through resource utilization and discharge planning.
Collaborate strongly with interdisciplinary teams (IDT).
Experience with MCG guidelines and discharge planning across acute care settings is required.

Required Qualifications

Valid RN license in Illinois
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Minimum 2 years of Acute Care Case Management experience, including experience within the last year
Experience in a Long Term Acute Care (LTAC) setting
BLS certification through the American Heart Association
Experience with MCG guidelines
