Description

Join a skilled nursing rehabilitation team as a Physical Therapist (PT) for a 13-week assignment working 8:00 AM – 4:30 PM with 35 guaranteed hours per week. The Physical Therapist will evaluate, develop, and implement individualized treatment plans for residents recovering from illness, injury, surgery, or functional decline. This role focuses on restoring mobility, improving strength and balance, enhancing independence, and maximizing overall quality of life through evidence-based rehabilitation services. Requirements: Minimum 1 year of Physical Therapy experience required
LTC/SNF or Rehab experience required
Active Physical Therapist License required
BLS Certification required
Experience working with geriatric and rehabilitation populations preferred
Strong patient assessment and treatment planning skills required
Ability to supervise PTAs, rehabilitation aides, and students required
Excellent communication, documentation, and interdisciplinary collaboration skills required
