Physical Therapist - Rehabilitation - (PT - Rehab)
StartDate: ASAP Available Shifts: 8 D Pay Rate: $2195.00 - $2331.00

Shift: M-F 8-4:30 rotating weekends Scrub Color/Attire - Grey scrub top and black scrub bottoms No floating Weekend coverage needed - Weekend Requirements: 1-2 weekends a month On Call Requirements None EMR – Meditech Type of equipment – Bioness; FES; Biodex Patient Ratios – 4-5/1 Number of Beds - 15 Utah Physical Therapy License; BLS Positive, upbeat, encouraging; engaging; compassionate; motivating Physical therapy evaluations with varying diagnoses 75-90 minutes physical therapy treatment plans; 25-30% of our patients are stroke/neuro Team player with all disciplines-PT/OT/ST/SW/RN/MD/CRS/PPSC Developing CARE scoring with all patients for quality/reimbursement purposes Number of Staff – 3 FT PT; 2 PRN PT; 4 FT OT; 2 PT OT; 3 PRN OT; 1 PRN COTA; 1 FT ST; 1 PRN ST Dept: IP Rehab Physical Therapy

Required Qualifications
Physical Therapist, Rehab
Experience: 2 years
PT(Compact Accepted)
Certifications: BLS-AHA
SSN Required
DOB Required
Work History: Should not be more than 90 days of gap in last 1 year

Good organizational and Communication skills working with the many disciplines

Preferred Qualifications
Neuro Certification
