Valid and unrestricted Physical Therapist license.
Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited program.
1 year of experience in skilled nursing facility- New grads welcome!
Strong interpersonal and communication skills.
Ability to work effectively in a team-oriented environment.
Commitment to providing high-quality patient-centered care.

Resume
Must have candidates current name, no nicknames, or abbreviations. Work history must include name of facility worked, city and state, title, department, and dates employed from the last 7 years. Skills Checklist

Two (2) Approved References:
Two (2) professional references from work history occurring within the past three (3) years. One must be from a supervisor (charge nurses are considered supervisors). Reference must include a breakdown of skills and strengths with a rating score. Letters of recommendation will not be accepted.
