Physical Therapist - Outpatient - (PT)
StartDate: 5/4/2026 Available Shifts: 8 D Pay Rate: $2326.00 - $2397.00

PT - Outpatient needed asap This facility will not consider any traveler who resides within 50 miles of facility. To avoid delays in processing, please provide perm address at profile upload. Requested time off during the course of this assignment MUST be noted and verified at upload. RTO REQUESTS AFTER UPLOAD WILL NOT BE APPROVED!! NO MORE THAN 5 DAYS OF RTO WILL BE ACCEPTED (less is better). Our Outpatient PT clinics are closed for major holidays. 2 years CURRENT PT experience - Required (New grads will not be considered for this need) BLS (AHA) Updated CAQH Required in profile NPI required in Profile Must be flexible - Will need to work Inpatient PT as needed Must be able to treat all normal diagnoses seen in outpatient clinics. Graduate from an accredited school of Physical Therapy - Required Previous Rehab Experience - Required Previous travel experience - REQUIRED - At least 3 mos. CPSI EMR experience - Highly Preferred Previous experience working with PTA's - Preferred This is a small community without public transport. Traveler candidate must have their own vehicle. This is a firm requirement.
