Travel PT Home Health jobs in San Francisco, CA start ASAP for a 13-week contract, working Monday through Friday with 8-hour days. You must have a California PT license and home health experience. You will conduct in-home physical therapy evaluations, develop individualized treatment plans, and implement therapeutic exercises to promote recovery and rehabilitation. You will monitor patient progress, adjust care plans as needed, and educate patients and families on treatment procedures and home exercises. Mileage reimbursement is provided at $0.72 per mile.
