This Home Health Physical Therapist opportunity is based in Durham, North Carolina, a dynamic city at the heart of the Research Triangle. In this role, the Home Health Physical Therapist provides one-on-one care in patients’ homes, focusing on functional rehabilitation, safety, and independence. Common responsibilities include post-acute rehabilitation following orthopedic surgeries or hospitalizations, gait and balance training, strength and endurance programs, fall risk assessments, home safety evaluations, caregiver education, and coordination of durable medical equipment. You will collaborate closely with nurses and other therapists to manage complex medical conditions, monitor progress, and adjust treatment plans. Visit volumes and caseload expectations are structured to support safe, high-quality care and to allow time for thorough documentation and communication. Most visits are scheduled during daytime hours on weekdays, with the possibility of occasional weekend or rotating coverage depending on agency needs. You will use a portable electronic medical record system to document visits in real time, which helps streamline workflow and support compliance with regulatory standards. Patient populations are primarily adult and geriatric, with a mix of post-surgical, neurologic, cardiopulmonary, and chronic disease management cases, offering excellent opportunities to develop and refine a broad clinical skill set.
