This Home Health Physical Therapist opportunity is based in Roseburg, Oregon, a welcoming community nestled in the heart of the Umpqua Valley. Roseburg offers a blend of small-town charm and access to incredible outdoor amenities, with scenic rivers, nearby waterfalls, and beautiful forested landscapes. Residents enjoy hiking, fishing, wine tasting at local vineyards, and a relaxed, family-friendly lifestyle. The area offers a reasonable cost of living, minimal commute times, and a close-knit community feel that makes it easy to put down roots or enjoy a rewarding travel assignment. The home health agency you will join is known for its patient-centered approach and commitment to high-quality, evidence-based care in the community setting. You will be part of a supportive, collaborative interdisciplinary team that includes nursing, occupational therapy, speech therapy, medical social work, and home health aides. The organization fosters a culture of open communication, clinical excellence, and continuous learning, with opportunities to participate in case conferences, in-services, and quality improvement projects. In this role, you will provide skilled physical therapy services to adult and geriatric patients in their homes throughout Roseburg and the surrounding area. Your typical day will include performing comprehensive evaluations, developing individualized plans of care, and delivering hands-on treatment focused on improving functional mobility, strength, balance, and safety in the home environment. You will educate patients and caregivers on home exercise programs, fall prevention strategies, safe transfers, and the proper use of assistive devices. You will manage a community-based caseload that may include patients recovering from surgery, stroke, cardiac events, fractures, and those living with chronic conditions such as COPD, heart failure, or neurologic diagnoses. The home setting allows for meaningful one-on-one treatment sessions tailored to the patient’s actual environment, giving you the opportunity to see the impact of your interventions on daily life. Visit volume is designed to support thorough, high-quality care, and you will coordinate closely with the rest of the care team to ensure patients receive comprehensive, integrated services. Schedules typically offer daytime hours with some flexibility to plan your day around patient needs and travel routes. You will travel between homes within a defined geographic territory around Roseburg, allowing you to become familiar with the community while minimizing extensive driving. Electronic medical record systems and mobile documentation tools support efficient charting and communication with your team. This position offers a chance to build or expand your home health experience while enjoying the lifestyle benefits of living and working in Roseburg. Whether you value the chance to practice autonomously in patients’ homes, develop expertise in community-based rehab, or simply enjoy the balance of a fulfilling career in a scenic Oregon setting, this role offers a rewarding professional path.
