This Home Health Physical Therapist opportunity is based in York, Pennsylvania, a historic community in south-central Pennsylvania known for its welcoming neighborhoods, affordable cost of living, and easy access to both rural beauty and larger cities. As a Home Health PT in York, you will provide one-on-one rehabilitative care to patients in the comfort of their homes, helping them regain independence, mobility, and safety. The role centers on evaluating patients after illness, injury, surgery, or functional decline; developing individualized treatment plans; and implementing evidence-based interventions that fit each patient’s home environment. You will perform comprehensive assessments, including strength and balance testing, gait and transfer analysis, home safety evaluations, and fall-risk screening. A typical day involves traveling within a defined service area around York to complete scheduled patient visits, with time allotted for documentation and care coordination. Daily visit volumes are designed to support quality, patient-centered care and reasonable travel time, often ranging from 5 to 7 visits per day depending on patient needs and geography. You will collaborate closely with an interdisciplinary team that may include nursing, occupational therapy, speech therapy, social work, and home health aides to ensure coordinated, holistic care. Regular communication with physicians and case managers is expected to review progress, update plans of care, and align on patient goals. Core responsibilities include conducting initial and reassessment visits, setting functional and measurable goals, providing therapeutic exercises and manual therapy as appropriate, designing home exercise programs, and recommending adaptive equipment and home modifications to improve safety. You will educate patients and caregivers on mobility strategies, energy conservation, fall prevention, and safe use of equipment such as walkers and transfer devices. The role requires careful attention to documentation within the agency’s EMR system to meet regulatory, accreditation, and quality standards in home health. Home health in York offers a high degree of autonomy and flexibility while still providing strong support. You will typically work day shift hours with some scheduling variations based on patient needs and agency expectations, often including occasional weekend or holiday coverage shared among the team. Caseloads are primarily adult and geriatric, with a mix of orthopedic, neurologic, cardiopulmonary, and general medical conditions commonly seen in a home health setting.
