This Home Health Physical Therapist role is based in Jeffersonville, Indiana, a vibrant riverfront community situated directly across the Ohio River from Louisville, Kentucky. As a Home Health Physical Therapist, you will deliver one-on-one, patient-centered care in patients’ homes throughout Jeffersonville and neighboring communities. You will have the opportunity to build meaningful relationships with patients and families, focusing on functional independence, safety, and quality of life in the environment that matters most to them. Typical diagnoses include post-operative orthopedic conditions, deconditioning, balance and gait disorders, neurologic conditions, and cardiopulmonary limitations. Your typical day will involve traveling between patient homes, performing initial evaluations, revisits, and discharge assessments. You will complete comprehensive assessments, establish individualized plans of care, and implement evidence-based therapeutic interventions to improve mobility, strength, balance, and functional independence. You will conduct home safety evaluations, recommend adaptive equipment or environmental modifications, and provide extensive education on home exercise programs, fall prevention, and safe mobility strategies. You will collaborate closely with an interdisciplinary home health team, which may include nursing, occupational therapy, speech therapy, social work, and home health aides. Clear communication and timely documentation in the EMR are essential to ensure coordinated, high-quality care. Patient caseload and daily visit expectations are structured to allow sufficient time for both hands-on treatment and thorough documentation, while supporting work-life balance. Schedules generally follow daytime hours with flexibility based on patient needs and agency requirements, and weekend or holiday coverage may be rotated among team members. This role is well-suited for a Physical Therapist who values clinical autonomy, enjoys working independently in the community, and is motivated by the opportunity to see patients progress in their own homes.
