This Home Health Physical Therapist opportunity is based in Ashland, Kentucky, a welcoming riverfront community situated along the Ohio River in the heart of the Tri-State area. As a Home Health Physical Therapist, you will provide one-on-one, evidence-based care to patients in their homes across Ashland and surrounding neighborhoods. This setting allows you to see firsthand how patients function in their daily environments and to design truly individualized treatment plans that support independence, safety, and quality of life. You will work with adult and geriatric patients recovering from surgery, illness, injury, or managing chronic conditions. Typical responsibilities include comprehensive physical therapy evaluations, development and implementation of goal-oriented treatment plans, and ongoing reassessment of progress. You will perform gait and balance training, strengthening and conditioning programs, functional mobility training, home safety assessments, and patient and caregiver education. You will also identify needs for durable medical equipment and modifications that support safe mobility in the home. Your day generally includes a manageable caseload of home visits scheduled within a defined geographic territory, with time allocated for thorough documentation in an EMR system. Visit volumes and daily structure are designed to support high-quality care rather than rushed encounters, allowing you to build meaningful relationships with your patients. You will collaborate closely with an interdisciplinary team that may include nurses, occupational therapists, speech therapists, social workers, and home health aides to coordinate comprehensive plans of care. Shifts are typically daytime hours on weekdays, with some flexibility based on patient needs and agency scheduling practices. Weekend or on-call rotations may be required depending on current caseload and staffing patterns, with scheduling communicated in advance. The role offers a high degree of autonomy in arranging your day, planning visit routes, and tailoring interventions while still having access to clinical leadership and peer support.
