As a Home Health Physical Therapist here, you will provide one-on-one care in patients’ homes throughout Pulaski and surrounding rural communities. You will perform comprehensive assessments, develop individualized plans of care, and deliver skilled interventions including strength and balance training, gait training, transfer training, and home safety modifications. Typical caseloads are designed to be manageable, with visit volumes tailored to support high-quality care and thorough documentation. You will travel between patient homes, often driving scenic country routes and working with a diverse patient population, primarily adults and older adults recovering from illness, surgery, injury, or managing chronic conditions. You will coordinate closely with an interdisciplinary team that may include nursing, occupational therapy, speech therapy, social work, and home health aides to ensure cohesive, patient-centered care. A typical day often includes morning and afternoon home visits, completion of EMR documentation, communication with physicians and care coordinators, and regular phone or virtual check-ins with your home health team. You will have the opportunity to exercise a high level of clinical autonomy, while still benefiting from a supportive, collaborative structure and clinical leadership. The environment is well-suited for clinicians who value flexibility, independence, and the ability to see the full impact of their work over time. Shifts are generally daytime hours on weekdays, with some agencies requiring occasional weekend or on-call coverage depending on patient needs. Your schedule may include a mix of evaluation visits, routine follow-up visits, and re-assessments based on regulatory guidelines and the progression of your patients. Because care is delivered in the home, there is significant emphasis on patient and caregiver education, fall prevention strategies, and practical, functional training tailored to each patient’s living situation.
