Physical Therapist - Home Health - (PT - HH)

This Home Health Physical Therapist position is based in and around Fort Mitchell, Kentucky, a welcoming suburban community in Northern Kentucky. The area offers a comfortable blend of small-town charm and big-city access, with tree-lined neighborhoods, excellent local schools and a strong sense of community. Fort Mitchell is just minutes from downtown Cincinnati, giving you easy access to major-league sports, vibrant arts venues, riverfront parks, live music, museums and a wide variety of restaurants and shops. Nearby recreation includes walking and biking trails, golf courses, local parks and seasonal festivals, making it an appealing location for professionals and families alike. You will provide skilled physical therapy services to patients in their homes across Fort Mitchell and neighboring Northern Kentucky communities. The typical caseload includes adult and geriatric patients with orthopedic, neurologic and cardiopulmonary conditions, post-surgical needs, chronic disease-related mobility issues and general functional decline. Your work will focus on helping patients safely remain in their homes, maximizing independence, reducing fall risk and improving quality of life. The role involves performing comprehensive home-based physical therapy evaluations, developing individualized plans of care, and delivering evidence-based interventions tailored to each patient’s goals and home environment. Common responsibilities include gait and balance training, strengthening and endurance programs, transfer training, home safety assessment, recommendation and instruction on assistive devices and durable medical equipment, and detailed education for patients and caregivers on home exercise programs and safe mobility strategies. You will collaborate closely with an interdisciplinary team that may include nursing, occupational therapy, speech therapy, medical social work and home health aides to coordinate care and ensure cohesive treatment plans. Communication with referring physicians and case managers is an important part of the role, as is contributing to ongoing care conferences and discharge planning when needed. A typical day includes traveling between patient homes in Fort Mitchell and surrounding communities, performing scheduled visits, documenting care in an electronic medical record system, and adjusting your route and visit timing as needed to meet patient needs. Drive time is a regular part of the role, and efficient route planning will help you manage your schedule. You will generally work within defined daytime hours, with shift patterns determined by agency scheduling needs and patient volumes, and you can expect a predictable home health workload with clear visit expectations. Patient ratios are structured around individual visits rather than unit-based staffing, allowing you to focus one-on-one with each patient during their scheduled home visit. This provides the opportunity for meaningful therapeutic relationships and tailored interventions that account for each patient’s unique living situation.
