This home health Occupational Therapist position is based in Somersworth, New Hampshire, a welcoming small city in the Seacoast region that blends historic New England charm with convenient modern amenities. As a home health Occupational Therapist in this role, you will provide one-on-one, patient-centered care in the comfort of patients’ homes. Your typical day may include performing comprehensive functional and home safety assessments, developing individualized treatment plans, and guiding patients through therapeutic activities designed to improve independence with daily tasks. You will help patients with bathing and dressing strategies, kitchen and household tasks, mobility around the home, and use of adaptive equipment and durable medical equipment. A significant part of your work will involve educating patients and caregivers on safe techniques, energy conservation, and modifications that support aging in place and recovery after illness or surgery. You will collaborate closely with an interdisciplinary home health team that may include nursing, physical therapy, speech therapy, social work, and home health aides. Together, you support patients recovering from hospitalization, managing chronic conditions, or transitioning from facility-based care back to the community. Patient acuity and complexity can vary, and caseloads typically focus on adults and older adults, with visit volumes tailored to regulatory standards and agency expectations. Patient ratios are structured around a manageable daily visit schedule to allow for thorough interventions and detailed documentation. In this position, you can expect a mix of scheduled home visits during daytime hours, with some flexibility in arranging your daily route across Somersworth and nearby towns. You will use an electronic medical record system and mobile documentation tools to complete evaluations, progress notes, and discharge summaries in a timely manner. The environment is well suited to clinicians who enjoy autonomy, problem-solving in real-world home settings, and forming meaningful relationships with patients and families over the course of care episodes.
